    In 1924, J. B. S. Haldane used the observation of increasing melanic frequencies in peppered moths (Biston betularia L.) to illustrate strong selection in a natural population. Since the phenomenon was first observed, there has been criticism and misinterpretation of work on industrial melanism in moths coming from a number of directions, increasingly on the Internet. Haldane’s calculation, its reception and his other interests in peppered moths are reviewed. An example of Internet comments attributing opinions to him, and their origin and background, are discussed

    Multimodal optical characterisation of collagen photodegradation by femtosecond infrared laser ablation.

    Collagen is a structural component of the human body, as a connective tissue it can become altered as a result of pathophysiological conditions. Although the collagen degradation mechanism is not fully understood, it plays an important role in ageing, disease progression and applications in therapeutic laser treatments. To fully understand the mechanism of collagen alteration, in our study photo-disruptive effects were induced in collagen I matrix by point-irradiation with a femtosecond Ti-sapphire laser under controlled laser ablation settings. This was followed by multi-modal imaging of the irradiated and surrounding areas to analyse the degradation mechanism. Our multi-modal methodology was based on second harmonic generation (SHG), scanning electron microscope (SEM), autofluorescence (AF) average intensities and the average fluorescence lifetime. This allowed us to quantitatively characterise the degraded area into four distinct zones: (1) depolymerised zone in the laser focal spot as indicated by the loss of SHG signal, (2) enhanced crosslinking zone in the inner boundary of the laser induced cavity as represented by the high fluorescence ring, (3) reduced crosslinking zone formed the outer boundary of the cavity as marked by the increased SHG signal and (4) native collagen. These identified distinct zones were in good agreement with the expected photochemical changes shown using Raman spectroscopy. In addition, imaging using polarisation-resolved SHG (p-SHG) revealed both a high degree of fibre re-orientation and a SHG change in tensor ratios around the irradiation spot. Our multi-modal optical imaging approach can provide a new methodology for defining distinct zones that can be used in a clinical setting to determine suitable thresholds for applying safe laser treatments without affecting the surrounding tissues. Furthermore this technique can be extended to address challenges observed in collagen based tissue engineering and used as a minimally invasive diagnostic tool to characterise diseased and non-diseased collagen rich tissues

    Clinical audit of dysphagia practices: Is the management of clients altered following videofluoroscopic examination?

    The videofluoroscopic procedure (VFSS) is currently considered the "gold standard" in assessing, diagnosing and informing the management of clients with dysphagia. The purpose of this investigation was to evaluate the clinical utility of the VFSS and whether it altered management of dysphagic clients. The present study replicated and extended previous research by Martin-Harris, Logemann, McMahon, Schleicher and Sandidge (2000). Files from one hundred inpatients with neurological disorders who had undergone a VFSS at the National Hospital of Neurology and Neurosurgery were reviewed. Data was obtained according to six variables dysphagia severity ratings, referral onwards, mode of intake alterations, diet consistency alterations, compensation strategies and swallowing therapy that improved the swallow. As found by Martin-Harris et al. (2000), over three quarters (82%) of the sample experienced change in at least one of the variables, with the majority experiencing change in more than two variables. The VFSS resulted in significant alterations in severity ratings, mode of intake, diet consistencies, in addition to identifying effective compensation strategies. However, no significant changes were found following VFSS in referral onwards or the implementation of swallowing therapy. These findings are discussed with reference to their external validity in light of the current limitations of the VFSS

    The impact of El Niño-Southern Oscillation (ENSO) on winter and early spring U.S. tornado outbreaks

    © 2017 American Meteorological Society. In recent years, the potential of seasonal outlooks for tornadoes has attracted the attention of researchers. Previous studies on this topic have focused mainly on the influence of global circulation patterns [e.g., El Niño-Southern Oscillation (ENSO), North Atlantic Oscillation, or Pacific decadal oscillation] on spring tornadoes. However, these studies have yielded conflicting results of the roles of these climate drivers on tornado intensity and frequency. The present study seeks to establish linkages between ENSO and tornado outbreaks over the United States during winter and early spring. These linkages are established in two ways: 1) statistically, by relating raw counts of tornadoes in outbreaks (defined as six or more tornadoes in a 24-h period in the United States east of the Rocky Mountains), and their destructive potential, to sea surface temperature anomalies observed in the Niño-3.4 region, and 2) qualitatively, by relating ENSO to shifts in synoptic-scale atmospheric phenomena that contribute to tornado outbreaks. The latter approach is critical for interpreting the statistical relationships, thereby avoiding the deficiencies in a few of the previous studies that did not provide physical explanations relating ENSO to shifts in tornado activity. The results suggest that shifts in tornado occurrence are clearly related to ENSO. In particular, La Niña conditions consistently foster more frequent and intense tornado activity in comparison with El Niño, particularly at higher latitudes. Furthermore, it is found that tornado activity changes are tied not only to the location and intensity of the subtropical jet during individual outbreaks but also to the positions of surface cyclones, low-level jet streams, and instability axes

    A web-based simulation of a longitudinal clinic used in a 4-week ambulatory rotation: a cohort study

    <p>Abstract</p> <p>Background</p> <p>Residency training takes place primarily on inpatient wards. In the absence of a resident continuity clinic, internal medicine residents rely on block rotations to learn about continuity of care. Alternate methods to introduce continuity of care are needed.</p> <p>Methods</p> <p>A web-based tool, Continuity of Care Online Simulations (COCOS), was designed for use in a one-month, postgraduate clinical rotation in endocrinology. It is an interactive tool that simulates the continuing care of any patient with a chronic endocrine disease. Twenty-three residents in internal medicine participated in a study to investigate the effects of using COCOS during a clinical rotation in endocrinology on pre-post knowledge test scores and self-assessment of confidence.</p> <p>Results</p> <p>Compared to residents who did the rotation alone, residents who used COCOS during the rotation had significantly higher improvements in test scores (% increase in pre-post test scores +21.6 [standard deviation, SD, 8.0] vs. +5.9 [SD 6.8]; p < .001). Test score improvements were most pronounced for less commonly seen conditions. There were no significant differences in changes in confidence. Residents rated COCOS very highly, recommending its use as a standard part of the rotation and throughout residency.</p> <p>Conclusion</p> <p>A stand-alone web-based tool can be incorporated into an existing clinical rotation to help residents learn about continuity of care. It has the most potential to teach residents about topics that are less commonly seen during a clinical rotation. The adaptable, web-based format allows the creation of cases for most chronic medical conditions.</p

    Use of mobile technology-based participatory mapping approaches to geolocate health facility attendees for disease surveillance in low resource settings.

    BACKGROUND: Identifying fine-scale spatial patterns of disease is essential for effective disease control and elimination programmes. In low resource areas without formal addresses, novel strategies are needed to locate residences of individuals attending health facilities in order to efficiently map disease patterns. We aimed to assess the use of Android tablet-based applications containing high resolution maps to geolocate individual residences, whilst comparing the functionality, usability and cost of three software packages designed to collect spatial information. RESULTS: Using Open Data Kit GeoODK, we designed and piloted an electronic questionnaire for rolling cross sectional surveys of health facility attendees as part of a malaria elimination campaign in two predominantly rural sites in the Rizal, Palawan, the Philippines and Kulon Progo Regency, Yogyakarta, Indonesia. The majority of health workers were able to use the tablets effectively, including locating participant households on electronic maps. For all households sampled (n = 603), health facility workers were able to retrospectively find the participant household using the Global Positioning System (GPS) coordinates and data collected by tablet computers. Median distance between actual house locations and points collected on the tablet was 116 m (IQR 42-368) in Rizal and 493 m (IQR 258-886) in Kulon Progo Regency. Accuracy varied between health facilities and decreased in less populated areas with fewer prominent landmarks. CONCLUSIONS: Results demonstrate the utility of this approach to develop real-time high-resolution maps of disease in resource-poor environments. This method provides an attractive approach for quickly obtaining spatial information on individuals presenting at health facilities in resource poor areas where formal addresses are unavailable and internet connectivity is limited. Further research is needed on how to integrate these with other health data management systems and implement in a wider operational context

    Classification of fracture and non-fracture groups by analysis of coherent X-ray scatter

    Osteoporotic fractures present a significant social and economic burden, which is set to rise commensurately with the aging population. Greater understanding of the physicochemical differences between osteoporotic and normal conditions will facilitate the development of diagnostic technologies with increased performance and treatments with increased efficacy. Using coherent X-ray scattering we have evaluated a population of 108 ex vivo human bone samples comprised of non-fracture and fracture groups. Principal component fed linear discriminant analysis was used to develop a classification model to discern each condition resulting in a sensitivity and specificity of 93% and 91%, respectively. Evaluating the coherent X-ray scatter differences from each condition supports the hypothesis that a causal physicochemical change has occurred in the fracture group. This work is a critical step along the path towards developing an in vivo diagnostic tool for fracture risk prediction
